I started the pilot hole with a center drill, then used the remaining portion of my long 1/4" drill bit to make the pilot hole penetrate both sides of the headrest.
Even though I made the drill rod pilot extra long, it ended up that even when only holding the hole saws hex shaped shank by 1/8" in the chuck of my drill, I was only able to drill about 80% of the way through the second side of the headrest. I was able to turn around the drill rod pilot and aligned the flat with the setscrew which in turn gave me a much longer shank on the hole saw! Instead of holding onto the hex shank of the holesaw with my chuck, I was grabbing onto the 1/4" round drill rod pilot. It's true there was no longer a pilot, but it ends up that I didn't need one because the hole saw was already 80% through, and the cut was good enough to act as a pilot. Worked great!
It took less time to make both holes than it did to describe the procedure.
